1. Medical & Lifestyle Questionnaire
Your GP will ask about:
Personal and family medical history
Lifestyle habits (e.g. diet, physical activity, smoking, alcohol use)
2. Physical Examination
Measurements may include:
Blood pressure
Cholesterol levels
Blood sugar levels
3. Risk Assessment
Your GP will calculate your cardiovascular disease risk using Heart Foundation Risk Assessment Tools.
4. Action Plan
You'll receive personalised recommendations:
