A Heart Health Check is a 15 to 20-minute consultation with your General Practitioner (GP) to assess your risk of heart attack or stroke within the next five years. It's a simple but powerful way to protect your long-term health.

1. Medical & Lifestyle Questionnaire

  • Your GP will ask about:

    • Personal and family medical history

    • Lifestyle habits (e.g. diet, physical activity, smoking, alcohol use)

2. Physical Examination

  • Measurements may include:

    • Blood pressure

    • Cholesterol levels

    • Blood sugar levels

3. Risk Assessment

  • Your GP will calculate your cardiovascular disease risk using Heart Foundation Risk Assessment Tools.

4. Action Plan

  • You'll receive personalised recommendations:
